Bitcoin Transaction Problem: PS2WSH Hex Code

The problem is with the PS2WSH hex code which is not allowing me to submit a new transaction. Here is the relevant information:

Unfortunately, when trying to transfer a PS2WSH transaction using the Bitcoin client or software, an error message appears indicating that the transfer failed.
